Global Youth Service Days Post-Project Information Form April 18-20, 2008

This form is only for projects which are launched, take place, or end during Global Youth Service Days (April 18-20, 2008). Please fill this form, directly after the implementation of your project, online: ww.avs.org.lb/gysd.html or fill it and send it by email to avs@avs.org.lb, or through the post: AVS, P.O. Box 136104, Beirut, or by fax: 01-449470. Be sure to fill in the information in both the Arabic and English. The deadline to submit this form is May 2.

Note: Please submit the project poster by 10 days after the implementation of the project and no later than May 9. Posters submitted after this date will not be eligible for the project awards competition.

هذه الاستمارة هي فقط للمشاريع الّتي ستطلق أو تقام أو تنتهي خلال الأيام العالمية للخدمة الشبابية (من 18 إلى 20 نيسان 2008. يرجى ملء هذه الاستمارة مباشرة بعد تنفيذ مشروعكم على صفحة الإنترنت: www.avs.org.lb/gysd.html أو يرجى ملئها وإرسالها عبر البريد الإلكتروني : avs@avs.org.lb أو عبر البريد: ص.ب. 136104، بيروت أو عبر الفاكس: 449470-01. إن المهلة الأخيرة لتسليم هذه الاستمارة هي 2 أيار

ملاحظة: يرجى تسليم ملصق المشروع بعد عشرة أيام من تنفيذه كحد أقصى، أما الملصقات التي ستسلّم بعد هذه المهلة فلن تكون مخولة لدخول المسابقة . المهلة الأخيرة لتسليم ملصق المشروع هي 9 أيار

3) Any local/national media coverage the event received? هل كان هناك أي تغطية إعلامية وطنية أو محلية للحدث؟
4) Any public officials and celebrities that supported your events in different ways (Example: being present, issuing a proclamation, sending a letter of support, etc.): - هل قدم لكم أي شخصيا ت رسمية أو معروفة أي نوع من الدعم - مثلا: زار المشروع خلال التنفيذ، أرسل رسالة دعم، أصدر بيانا الخ
5) Any sponsors who provided funding, grants, or in-kind donations that deserve recognition:  هل من راعين للمشروع قدموا أي تمويل، منح أم تبرعات عينية تستحق الشكر والتقدير؟
